Application for Workplace Vocational Training (Deadline: September 8, 2025)
For Workplace Training applications, the following documents must be submitted to the Department Chair by September 8, 2025:

SGK Eligibility Inquiry Document

Workplace Training Agreement (3 copies)

Workplace Training Student Acceptance Form (2 copies)

💡 How to obtain the SGK Eligibility Inquiry Document?

Log in to the e-Government (e-Devlet) system.
Type “SPAS Eligibility Inquiry (Health Provision Activation System)” in the search bar and select it.
Click on the "Generate Document with Barcode" tab on the opened page.
Click on the "Download File" tab and download the document as a PDF, then print it.
